These appointments are usually with a medical assistants work alongside the nurse practitioners in the SHC. More information on medical assistants.

IMPORTANT INFORMATION FOR YOUR APPOINTMENT

Arrive on time whether it's in person or via Zoom with forms complete. It is very important that each patient arrives at their scheduled time with their patient portal forms completed.  If you are late you may need to be rescheduled.

Insurance card

Enter your insurance information on the patient portal. Upload a copy of the front and back of your insurance card (If you cannot find your insurance name, call the SHC and we will help). It is still a good idea to bring your insurance card with you to every appointment. If you do not have insurance, you are still able to utilize the Health Center services. Please let the receptionist know when you schedule your appointment if you do not have health insurance.

All students pay a $5 flat visit fee to see a nurse practitioner. There is not an administrative fee charged to for a Vaccine/Lab only appt. You may have additional costs depending on the services received - you will be informed of the cost prior to the service being provided.

If you are unable to make it to your scheduled appointment, we require 4 hours’ notice due to the high demand for appointments. If you miss your appointment or don't give appropriate notification it will be considered a Missed Appointment which carries a $20 charge.  You have the option of cancelling your appointment on the patient portal as long as it's more than 4 hours before your appointment.

To complete your forms, you will log onto the Patient Medical Portal using your SU email username and password.

On the home page you will see your appointment and a green Forms button. Click on the green button to access the forms required for your upcoming appointment. You can also click on Forms which will show the required forms for the appointment.
